Transcript and Diploma Information

Transcript Requests

If you would like to request an academic transcript please download the Transcript Request Form or submit a written request to the Registrar's office. If submitting a written request, please include the following information:

Full Name (Maiden, if applicable)
Current Address & Phone Number
Social Security Number
Dates Attended
Date of Birth
Major
Address of Receiver
Signature

Signed Transcript Request Forms can be faxed to 419.998.3080. The signed forms can also be scanned and submitted by email to advisor@unoh.edu.

Please Note:

  • There is no charge for transcripts.
  • Transcripts are processed each Friday.
  • Requests delivered after noon on Friday may have to wait until the next week.
  • Transcripts being sent by mail will be put into the University's next delivery of outgoing mail as they are processed on Friday.
  • Rush orders require a $30 fee.
  • Transcripts cannot be processed if the student has a financial hold on an account.
  • Fill out separate request for each address.

Degree Printing

If you would like to request a reprint of your degree certificate, please submit a written request and include the following information:

Full Name (Maiden, if applicable)
Current Address & Phone Number
Social Security Number
Dates Attended
Date of Birth
Major
Signature

Degree certificate reprints should include a processing fee of $15.00 made payable to the University.

Academic records cannot be released without the written consent of the student.
